Requests for Transcripts
You may request transcripts by filling out and sending our Transcript Request Form by Fax to (606)666-5422, Attn: Principal. NOTE: Form must be signed and dated. (Trouble downloading? Follow these instructions: Right-click Transcript Request and select Save-as, or Ctrl-click and Download linked file on the Macintosh )
You may also request transcripts by filling out and sending a Transcript Request Form signed and dated, to:

If you are not able to print out the Transcript Request Form, you may also send a written request that includes the following information:
Name under which the student attended
Social Security Number
Address(es) to which the transcript(s) is to be mailed
Current mailing address
PLEASE NOTE: We do not accept telephone or e-mail requests.
We will grant two transcripts for any graduates or former students within two years of their graduation or transfer from Oakdale.
There will be a $10 fee for requests that have exceeded either the two transcripts or two-year limit. Each additional requested transcript will incur a $10 fee.
Make checks payable to Oakdale Christian Academy, Attn: Business Office.
All transcript requests will be processed within 5 business days. If you need a transcript processed faster than 5 business days there will be an additional $5 fee.
