Parent-Student Handbook
Applications may be faxed to 606-393-0200. Send the $30 Application Fee payment.
U.S. Student Application Requirements
- Complete and return the parent and student pages of the handbook.
- Request school transcript and the Principal Recommendation from your current school. Please use the forms in the application.
- Ask a professional of your choice (teacher, counselor, coach, youth worker, pastor) to complete and return the Professional Recommendation form.
- Obtain a doctor’s physical with a copy of your immunization record. Please use form in the application.
- Applications are considered complete when Oakdale has received all pages of the application forms, medical forms, school transcript, recommendation forms and application fee.
- Some applicants may be required by the admissions committee to visit campus prior to the committee making an acceptance decision.
Enrollment
Send the $250 Enrollment payment here. The enrollment fee is to reserve your seat at Oakdale Christian Academy.
