You may request transcripts by filling out and sending our Transcript Request Form by email to principal@oakdalechristian.org. Attn: Principal. NOTE: Form must be signed and dated.
By Mail
You may also request transcripts by filling out and sending a Transcript Request Form signed and dated, to: Oakdale Christian Academy Attn: Academic Department 5801 Beattyville Rd. Jackson, KY 41339 If you are not able to print out the Transcript Request Form, you may also send a written request that includes the following information:
Name under which the student attended
Social Security Number
Address(es) to which the transcript(s) is to be mailed
Current mailing address
Phone number
Signature
PLEASE NOTE: We do not accept telephone or e-mail requests. We will grant two transcripts for any graduates or former students within two years of their graduation or transfer from Oakdale. There will be a $10 fee for requests that have exceeded either the two transcripts or two-year limit. Each additional requested transcript will incur a $10 fee. Make checks payable to Oakdale Christian Academy, Attn: Business Office. All transcript requests will be processed within 5 business days. If you need a transcript processed faster than 5 business days there will be an additional $5 fee. For further information call 606-332-0432, or e-mail Laura Mead.
